Upon admission to L.V. Stabler Memorial Hospital, you will be given a form, “Consent for Disclosure of Information to Clergy”. You may choose “I Do Consent” or “I Do Not Consent” on the form.  If you choose "I Do Consent" we will notify your clergy on your behalf. If you do not have a specific person, we will provide you with a list of clergy who have agreed to assist with spiritual care for our patients.
